Centre for Health Innovation fellowship visit 

HealthTag เครือข่ายเชื่อมโยงข้อมูลสุขภาพ ผู้ใช้งานเป็นศูนย์กลาง ยกระดับระบบการรักษาพยาบาลในไทย โดยมีแนวคิดเปลี่ยนรูปแบบข้อมูลจากแบบรวมศูนย์ (ศูนย์กลางอยู่ที่สถานพยาบาลแต่ละแห่ง) มาเป็นแบบกระจายศูนย์ (ศูนย์กลางอยู่ที่ตัวบุคคล) ด้วยการนำเทคโนโลยีบล็อกเชนมาประยุกต์ใช้ ทำให้ผู้ใช้งานสามารถเป็นเจ้าของข้อมูลสุขภาพของตนเองอย่างแท้จริง

HealthTAG คือ เครือข่ายเชื่อมโยงข้อมูลสุขภาพ โดยมีกุญแจฝังอยู่ในชิปเข้ารหัสแบบพิเศษที่ใช้เป็นตัวกลางในการเข้าถึงข้อมูล เป็นบัตรประจำตัวสุขภาพอิเล็กทรอนิกส์ คล้ายกับบัตรประชาชนด้านสุขภาพที่บันทึกข้อมูลสุขภาพส่วนบุคคล (Personal Health Record) ที่อยู่บนพื้นฐานของความปลอดภัยทางข้อมูล (Data Privacy) ซึ่งคนที่มีสิทธิเก็บและเข้าถึงข้อมูลได้คือ บุคคลผู้เป็นเจ้าของ กับ บุคลากรทางการแพทย์ที่ได้รับอนุญาติให้เข้าถึง

โครงสร้างที่เกิดขึ้นทำให้เกิดทางด่วนข้อมูลเส้นใหม่ ที่มีความเป็นอธิปไตยและเป็นโครงสร้างแบบเปิด ที่นอกจากตัวผู้ใช้และสถานพยาบาลจะได้รับประโยชน์แล้ว หน่วยงานที่เกี่ยวข้อง หรือแพลตฟอร์มต่างๆก็สามารถมาใช้งานได้ เกิดเป็น Ecosystem แบบใหม่ที่ตั้งอยู่บนความเป็นส่วนตัวและมีผู้ใช้เป็นศูนย์กลางอย่างแท้จริง

## ไทม์ไลน์ HealthTAG
HealthTAG เริ่มพัฒนาต้นปี 2563 ตรงกับช่วงที่โควิด-19 ระบาดหนักพอดี ได้มีโอกาสไปเสนอโครงการสาธารณสุขจังหวัด ได้รับความสนใจอยู่ไม่น้อย แต่สุดท้ายเมื่อโควิดเงียบลง (ในตอนนั้น) หน่วยงานภาครัฐก็เงียบตามไปด้วย ทำให้เช้าใจว่าการจะผลักดันระบบให้เกิดขึ้นไม่ใช่เรื่องง่าย

หลังจากนั้น HealthTAG ได้พัฒนาต่อมาจนเป็นรูปเป็นร่างมากขึ้น พร้อมกับความเชื่อที่ว่าโลกของข้อมูลคือสิ่งสำคัญ การบันทึกข้อมูลสุขภาพเป็นสิ่งจำเป็น จนได้ไปนำเสนอโครงการที่โรงพยาบาลศิริราช และโรงพยาบาลในเครือทั้ง โรงพยาบาลศิริราชปิยะมหาราชการุณย์ และศูนย์การแพทย์กาญจนาภิเษก ซึ่งสนใจและซื้อไอเดีย

นอกจากนี้ยังได้รับการสนับสนุนจากคณะกรรมการ 5G และ กสทช. ที่จะทำโครงการ Smart Hospital เพื่อเป็นต้นแบบก่อนขยายผลไปเครือข่ายกลุ่มโรงเรียนแพทย์ซึ่งมีโรงพยาบาลกว่า 20 แห่ง จึงได้มีโอกาสพูดคุยและต่อยอดไปยังโรงเรียนแพทย์ในหลายๆภูมิภาค

ตลอดปี 64 สถานการณ์โควิดมีทั้งช่วงที่หนักขึ้นและเบาลง ในการพัฒนาผลิตภัณฑ์ช่วงแรกเน้นในที่ข้อมูลโควิด แต่จากนั้นได้เรียนรู้ว่า หัวใจสำคัญหลักของ HealthTAG อยู่ที่การเก็บข้อมูลสุขภาพที่เชื่อมโยงกับสถานพยาบาลมากกว่า

จากนั้นในปี 65 จึงเริ่มเป็นที่รู้จักมากขึ้น มีโรงพยาบาลหลายแห่งใช้ผลิตภัณฑ์ HealthTAG เชื่อมโยงข้อมูล เช่น ข้อมูลยา การแพ้ยา ประวัติการรักษา และสิทธิการรักษา โดยไม่ต้องถามประวัติซ้ำหลายรอบ เพียงแค่คนไข้มีบัตร คุณหมอใช้ระบบโรงพยาบาลเดิม เชื่อมโยงข้อมูลถึงกันได้ตั้งแต่การรักษา การรับยา และการเบิกเคลมจ่าย ถ้าทุกอย่างเป็นไปตามแผน กลางปี 66 จะเร่ิมมีการใช้งานแพร่หลายมากขึ้นในโรงเรียนแพทย์หลายแห่ง และจากนั้นในอีก 2-3 ปี เชื่อว่าการใช้งานจะขยายวงกว้างออกไปมากขึ้น

สำหรับการทำงานของ HealthTAG เป็นแบบสตาร์ทอัพ ที่พัฒนาขึ้นมาโดย “หมอ” ที่เข้าใจปัญหาอย่างแท้จริง และมีความต้องการผลักดันให้ MedTech Ecosystem ในประเทศไทยเติบโต ที่สำคัญมาอย่างถูกที่ถูกเวลา ในช่วงที่มีการระบาดของโควิด-19 เป็นความจำเป็นของข้อมูลการรักษาพยาบาลที่ต้องเชื่อมถึงกัน

ล่าสุดเตรียมระดมเงินทุนเพื่อขยายบริการให้มากขึ้น ตอกย้ำจุดยืนการเป็น MedTech ที่มาจากฝีมือ ความคิด และการทำงานจริงของแพทย์ไทย อยากให้ทุกคนมองเห็นประโยชน์ตรงกัน 4 ส่วน

คนไข้: ได้รับการบริการที่สมบูรณ์มากขึ้น ลดความผิดพลาด ลดการตกหล่นของข้อมูล ได้เห็นประวัติสุขภาพและเห็นการเปลี่ยนแปลงแบบเรียลไทม์

บุคคลากรทางการแพทย์: ลดกระบวนการทำงานที่ซ้ำซ้อน ลดขั้นตอนเกี่ยวกับเอกสาร โฟกัสกับการรักษาได้มากขึ้นและแม่นยำขึ้น

ระบบสาธารณสุข: มีระบบการรักษาในประเทศแบบไร้รอยต่อและมีประชาชนเป็นศูนย์กลาง อีกทั้งยังเชื่อมโยงกับคนไข้ต่างชาติได้ง่ายขึ้น รายได้เข้าประเทศ สร้างความมั่นใจให้คนไข้ทั้งไทยและต่างชาติ

สตาร์ทอัพ: เกิด Open Data หรือข้อมูลที่มาจากการได้รับความยินยอม ในการนำไปพัฒนานวัตกรรม ทางการแพทย์ง่ายขึ้น

DEPA ผนึกกำลัง HealthTAG พัฒนาโครงสร้างพื้นฐาน Blockchain สำหรับบริการประชาชน

12 มกราคม 2566, สำนักงานส่งเสริมเศรษฐกิจดิจิทัล (depa) โดย ผศ.ดร.ณัฐพล นิมมานพัชรินทร์ ผู้อำนวยการใหญ่ และ ดร.ภาสกร ประถมบุตร รองผู้อำนวยการใหญ่ ลงนามในบันทึกข้อตกลงความร่วมมือ (MoU) เพื่อพัฒนาโครงสร้างพื้นฐาน Blockchain สำหรับบริการประชาชน ร่วมกับ บริษัท เฮลธ์แท็ก จำกัด (HealthTAG) โดย นายเดโชวัต พรมดา ประธานกรรมการบริหาร และนางสาวนวรัตน์ ติณสุข กรรมการผู้อำนวยการฝ่ายปฏิบัติการ

ความร่วมมือดังกล่าวมีวัตถุประสงค์เพื่อพัฒนาโครงสร้างพื้นฐานดิจิทัลด้านข้อมูลแบบกระจายศูนย์ด้วยการประยุกต์ใช้เทคโนโลยี Blockchain สำหรับบริการประชาชนหรือสาธารณะบนพื้นฐานความมั่นคงปลอดภัยของข้อมูล ความเป็นส่วนบุคคลของข้อมูล และการมีส่วนร่วมของทุกภาคส่วน นอกจากนี้ยังเป็นการส่งเสริมการแลกเปลี่ยนองค์ความรู้กับผู้เชี่ยวชาญ เพื่อพัฒนาบุคลากรด้านการประยุกต์ใช้เทคโนโลยี Blockchain รวมถึงส่งเสริมความร่วมมือระหว่างหน่วยงานภาครัฐ ภาคเอกชน ภาคประชาชน ทั้งในระดับท้องถิ่น ระดับประเทศ และระดับนานาชาติ

เทคโนโลยี Blockchain ในที่นี้หมายถึงเทคโนโลยีการจัดเก็บข้อมูลแบบ Shared Database หรือ Distributed Ledger Technology (DLT) โดยเป็นรูปแบบการบันทึกข้อมูลที่รับประกันความปลอดภัยว่า ข้อมูลที่ถูกบันทึกไปก่อนหน้านั้นไม่สามารถเปลี่ยนแปลงหรือแก้ไข ซึ่งผู้ใช้งานจะได้เห็นข้อมูลชุดเดียวกันทั้งหมด โดยใช้หลักการ Cryptography และความสามารถของ Distributed Computing เพื่อสร้างกลไกความน่าเชื่อถือ จุดแข็งหรือประโยชน์ของการจัดเก็บข้อมูลแบบกระจายส่วน (Distributed Ledger) ทั้งในแง่ความปลอดภัย ประสิทธิภาพการดำเนินการจัดเก็บข้อมูล และความรวดเร็วของการทำธุรกรรมโดยปราศจากตัวกลาง ซึ่งเหนือกว่าระบบเทคโนโลยีแบบรวมศูนย์ (Centralized Server-based System) ที่กำลังใช้กันอย่างแพร่หลายในปัจจุบัน โดยจะเริ่มนำร่องที่การให้บริการกระเป๋าเก็บเอกสารรับรองดิจิทัล (Digital Certificate Wallet) ด้วยการทำ Self-sovereign ID (SSI Wallet) ควบคู่กับ Decentralized Identifier (DID) เพื่อให้ประชาชนสามารถเก็บเอกสารรับรองต่าง ๆ

ผศ.ดร.ณัฐพล กล่าวว่า การให้บริการโครงสร้างพื้นฐาน Blockchain ที่ร่วมกันพัฒนาขึ้นนี้จะช่วยให้ประชาชนสามารถเป็นเจ้าของข้อมูลของตนเองอย่างแท้จริง เสมือนมีข้อมูลของตนที่ติดตัวไปได้ทุกที่ และอัพเดตถูกต้องตลอดเวลาบนพื้นฐานของความมั่นคงปลอดภัย และความเป็นส่วนบุคคลของข้อมูล รวมถึงให้คำแนะนำที่เป็นประโยชน์แก่หน่วยงานภาครัฐและภาคเอกชนที่มีศักยภาพต่อไปในอนาคต
